Immigration Physicals

What To Bring To the Office

1. Your I-693 Form filled our with BLACK INK or TYPED. DO NOT sign or date at home, this must be done at the office when directed!

2. Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name.

3. Your Vaccination/Immunization Record if available, including your COVID-19 vaccination card.

After Your Physical Is Complete

Once your I-693 Form is complete, we will seal the form in an envelope for you to submit to USCIS. DO NOT open or fold the envelope once sealed! We will provide you with a copy for your record.
